This Consent & Model Release Form (the "Form") is entered into by and between:

The Photography Company: R2A Visuals

1. Consent to Photography

I, the undersigned, hereby give my consent for [Photographer's Name] and/or R2A Visuals to take my photograph. I understand that my image will be captured during the photography session held on [Date of Shoot].

2. Model Release

I hereby grant R2A Visuals and its legal representatives and assigns the irrevocable and unrestricted right to use and publish photographs of me, or in which I may be included, for editorial, trade, advertising, and any other purpose and in any manner or medium, to alter the same without restriction, and to copyright the same.

I understand that the images may be used for, but not limited to:

Photographer's professional online portfolio

Photographer's website and blog

Social media platforms (e.g., Instagram, Facebook, Youtube)

Advertising and promotional materials (e.g., brochures, flyers, ads)

Photography contests and exhibitions

3. Waiver of Compensation

4. Permission for Minors (if applicable)

If the subject of the photographs is a minor, the undersigned parent or legal guardian acknowledges that they have the authority to enter into this agreement on behalf of the minor.

I have read and fully understand the terms of this Consent & Model Release Form. I voluntarily sign this form, and I am of legal age and competence to do so.
